Scores
Answers are graded on a 5-point scale. Each question yields a score of 0 (absent), 1 (sometimes) or 2 (regularly or more often). Subscale scores: sum of relevant item scores per subscale.
Standards
3 norm categories per subscale: Low, Moderately elevated, Highly elevated.

This questionnaire concerns various complaints and symptoms that you may have. It always concerns complaints and symptoms that you have experienced in the past week (the past 7 days including today). Complaints that you had before, but no longer had in the past week, do not count.
