CSHQ - Sleep Habits Questionnaire

Goal
Questions children's sleeping habits and possible problems with sleeping.
Scales
8 subscales: Not wanting to go to bed, Falling asleep late, Not getting enough sleep (sleep duration), Sleep anxiety, Staying awake at night, Parasomnia (including sleepwalking, nightmares, etc.), Abnormal breathing and Sleepiness during the day.
Scores
Items are rated on a 3-point scale (1-3). Some items are reverse scored. Total score: sum of all item scores.Subscale scores: sum of item scores of the relevant subscales.
Standards
3 standard categories: problematic, average, good. Cut-off point: 44. Standards are based on the non-clinical population.
Instructions
The following statements are about your child's sleeping habits and possible problems with sleep. Think back to how your child fared last week when answering the questions. If the past week has been different than normal (e.g. due to an ear infection that caused your child to sleep poorly), choose the most normal previous week. You can choose from the following answers: MOSTLY if something happened 5 or more times in 1 week SOMETIMES if something happened 2-4 times in 1 week RARELY if something never happened or happened 1 time in 1 week. Please also indicate whether the sleeping habit is a problem by checking YES, NO or NOT APPLICABLE.
