Goal
Observation list to evaluate problems in a client's daily functioning and to assess care needs.
Scales
4 subscales: gedragsproblemen, beperkingen, symptomen en sociale problemen.
Scores
Items are scored on a 5 scale (0 - 4).Total score: sum of all item scores.Subscale scores: sum of the item scores of the relevant subscales.
Standards
3 standard categories: low, medium, high. Threshold value for intensification of care. Standards based on clinical population (outpatient, day treatment & residential).
Instructions
Rate mental health status using the following 12 items. Do not use information that has already been included in a previous item, except for item 10, which is a global score. Rate the MOST SERIOUS problem that occurred in the past 2 weeks. Choose the answer option that most applies.
